Many stage changes do NOT send email notifications

Description
I have created a page and one content element in workspace. I am the workspace owner and member at the same time so all emails will come to me.
I changed to workspace module and sent all changes to the next stage using the arrows to the right on every item in the list. On every change I got the popup to send the notification email to myself which i confirmed. At the end I only got a few of them. Some stage changes just don't send an email...

I can confirm this issue but only regarding the last step "Publish".

Thanks for your report. Can you please tell which stages of the "some stages" are affected (only publish)? Which TYPO3 version do you use (I guess 4.5)?

Yes, only the publish stage is affected. I use 4.5.2

I also can confirm this issue, only the last step "publish" is affected (Typo3 4.5.3)

That is true. On the last step "Publish" no emails are send at the moment.
But can you please describe on which situation emails are mission beside the last step?

With 4.5.13 I can confirm this issue.
It seems that the last (publish) stage and custom stages are affected.
I applied patch http://review.typo3.org/4519 but it does not resolve the problems for me. Can anybody confirm this?

The workspace notifitations are not working if [MAIL][defaultMailFromName] in install tool is set.
Clearing my custom MailFromName solved the problem.

With Typo3 4.6.15 email are not send when the stage is send to a stage I've add in custom workspace.
When the content is send to stage "ready to publish" an email is send ...
