I think the email should only be sent the first time.
I looked for a way to set some kind of condition to the "setfixed.approve"-section in the Typoscript-setup, but as far as I can tell there seems to be no option in the included fe_adminLib.inc to do that.
Maybe it would make sense to generally show an error message when you try to change a setting via setfixed-links to a value that is already set.
