Remake of BE login (tableless, valid code,...)

Description
The following patch remakes the whole BE login with following features
- fieldset, labels, divs instead of tables
- valid code
1.) Because this patch will be introduced with 4.3, the attached changes are just a draft (logout not finished).
the file typo3\sysext\t3skin\stylesheets\typo3-index-php.css can be deleted because it has always been merged into the global typo3/stylesheet.css
2.) skinning can be easily done by the extension chloginskin which will be released ~ 16.3.08

Hey Georg,
nice work. Let's first get the templating (see #18864) in the core, then let's rework your patch to remove the tables. sounds good?

Hey Georg,
the Template changes are now in the trunk. We now need to strip out the markup changes and then the style changes. Do you have any time soon to look into it?

This is related to #20640 which introduces labels, divs etc. for the login-page.
It has been committed as Rev. 5899.
So is there anything left to do here?

Resolved, no change required:
This has been resolved with #20640 which reworked the whole login screen.
Please open new issues if there are still unhandled problems.
