Is typo3/ext still needed?
I haven't seens many projects where global extensions were installed.
Also putting extensions into typo3/ subfolders makes upgrading sources complicated.

I think this feature is still used, there was a discussion in the core list as well on this topic. There are agencies who deploy a modified core delivering a number of extensions together with the core for multiple instances at once. Codewise it is also acceptable to still handle that.
