Remove support for "global" extensions

Description
The functionality of "global" extensions,
which are extensions that reside under typo3/ext/
is removed, as this concept is not usable
anymore and has a lot of side effects (core
self update functionality etc).
The option
$TYPO3_CONF_VARS['EXT']['allowGlobalInstall']
is removed as well, as it is not needed anymore.

not solvable in a good way as the feature is still used, see forums and gerrit for more infos.
