Log FE usernames and also log failures

Description
FE usernames should be logged using config.stat_apache.
The attached patch does much more. It also logs server errors (404, 503, etc.) as far as they get processed by TYPO3. However, since the logfile is defined through TypoScript but server errors usually don't relate to a specific template, this logfile would have to be defined on a more global perspective. So this patch is just a prototype and still needs to be completed...

as this issue is very old. Does the problem still exists within newer versions of TYPO3 CMS (4.5 or 6.1)?

I don't find code in this direction, the patch is outdated. ;-) So closing this issue.
If you think that this is the wrong decision or experience this issue again, then please write to the mailing list typo3.teams.bugs with issue number and an explanation or open a new ticket and add a relation to this ticket number.
