When DAM 1.1 hooks on the RTE link insertion dialogue, some controls of the DAM media tab do not work.

Some DAM css and JavaScript are missing on the page. The attached patches fix the issue in SVN TYPO3core branch TYPO3_4-2 and trunk respectively. A patch is also required in SVN DAM trunk.
