I think this is very confusing. If Images are disabled in extConf the button should be not in toolbar.
I myself was confused very often that the image dieappears with saving until i noticed that this setting was active.

Regarding trunk, the image button will show up only if enableImages is set.

But, as stated in the manual, this should apply only to the Minimal and Typical default configuration. In the Demo default configuration, images should always be enabled.

The attached patch modifies the text of the enableImages property so as to make this clearer in EM. Also correcting trunk so that images are always enabled in Demo default configuration.

The same logic is back-ported to TYPO3 4.2 branch which should also fix the reported issue.
