Old Titles Show Up Instead of Newer Version

Description

We have three websites running with typo3 but there is one that is giving us an issue. In this website for some reason the old (first draft version) title is what is being displayed in the live version. When the cache is refreshed it does not help. The only way to fix it is to sign into typo3 and refresh every page one by one. This fixes the issue temporary but then reverts to the old titles again overnight. We have looked everywhere and we cant seem to figure out what may cause this. It seems to only read the first draft title and disregard any version after that.

Another way to fix this issue was to go to the first draft version and change the title there. This is a temp patch since the titles change from time to time and that update would not take effect.

Even when we change the title in the live version it still does not take effect. We have tried it in the new typo3 version and older versions and they all have the same issue.

Has anyone encountered this issue before? Is there a fix?

Sometimes after refreshing every page it can last up 2 days fine but usually within 24 hours it reverts.

The issue is very old, does this issue exists in newer versions of TYPO3 CMS (4.5 or 6.1)?
