CSH tooltips show also with context_help ext disabled and user option

Description

with context_help ext disabled i don't get the tooltips in the TCE forms, but they are still everywhere else (list and page module, user setup...)

also the user option "Show help text when applicable" isn't repected..
i would expect it to remove all those "?" icons around and tooltips on most interface elements

The bug is continued in TYPO3 4.5.2.

Help and tooltips can not be disabled with "helpText=0" and disabled context_help extension.
