Preview from workspaces sometimes successfull, but mostly failing

Description

Preview from workspaces is sometimes with success, but but mostly failed. This is a big problem for us. We are using frameset for our Intranet, however do not know, if this is relevant. We use 4.5.5

I attach a word document, where the issue can be seen, and I attached a web-log for both cases (successed preview and failed preview)

Sorry I don't want to be too harsh. But that's not a way to report bugs.

Please tell us 1) what the problem is and 2) How to reproduce it in other setups (e.g. with the Introduction package)
