Unable to create Elements in Form Wizard

Description
Way to reproduce:
- create a new mail-form element on a page
- save it once (otherwise the wizard icon is not shown up)
- enter the form wizard
- drag 'n drop an element to the right side
- start to fill up the options
suddenly the element disappears from the right side (or if you store)
after that the drag 'n drop functionality is broken and you cant add any element anymore
testet with FireFox 26 (I know) and Chromium 32 under Linux

Works fine here on OSX and on Ubuntu14 (KDE)
