If you don't check "Show secondary options (palettes)" you get a broken form on top of the page. Neither in IE nor in Firefox the form fits into the small (and somehow forlorn looking) grey block.
But not only is the block too small there real issues with overlaid (?) checkboxes.

I can confirm this. Looks the same here. The error occurs also on Firefox on linux.
