I can confirm this for Typo3 4.2.8 and php 5.2.10, too.
It happens, when Install Tool > All Config. > [GFX][noIconProc] = 1.
(It should be set to "0", see next note for details)

The wrong icons are displayed for these page types:
-Shortcut
-Mountpoint
-External Link
whenever
-include subpages and
-access > show at any login
is selected.

If [x] hide page is checked, even more wrong icons are displayed.

If you want all the test pages for your system, use the attached t3d-File "T3D__testpages_pagetree_icons.t3d".

I would suggest to set this issue to "solved"

Problem:
No more missing/wrong Icons in Backend > Pagetree

Solution:
Set Install Tool > All Config. > [GFX][noIconProc] = 0

Proof:

Versions:
Typo3 4.28, PHP 5.2.10

Hint:
If you still want custom icons in your pagetree, have a look at this
typo3 extension: kb_page_icon

Resolved, no change required.

Most of this depends on [GFX][noIconProc].
