RTE hangs on load in repetitive TV data object

Description
When opening an existing FCE with repetitive data object containing an RTE field the loading of the RTE failes (debug output below) "Editor is beeing loaded" / "Editor wird gelaed. Bitte warten".
The JS console shows:form is undefined

When adding a new data object the RTE is shown and is functional in the new object, but after saving the error seems to stop the rendering process with the first RTE.

Seen in: IE8 / FF 3.6.13 / Chrome 9.0.597.98 and in Typo3 4.5.0 as well!

Same problem here with TYPO3 4.5.1.
I provided a file Sample.zip which hopefully contains all files which are used for creation of a Templavoila FCE with which the error occurs. If you need further information please contact me.

I'm having a similar issue and I just upgraded to Typo3 4.5.2. It happens when I try to edit a record for the extension Calendar Base (cal). It even happens when I try to create a new record.

as this issue is very old. Does the problem still exists within newer versions of TYPO3 CMS (6.2.4)?
