config.absRefPrefix = /

is used, the slash will not be prefixed to the image URLs in the admin panel.

Step by step instructions to reproduce the problem in TYPO3 6.2:

install realurl
activate .htaccess
create a page and a subpage
create a template (setup see below)
go to the Frontend and enable all settings except "Explain SELECT queries" in the TypoScript section of the admin panel
now navigate to the subpage (e.g. http://myurl.tld/subpage/)
The icons in the tree are missing (see screenshot)

This should be in your template setup:

config.tx_realurl_enable = 1
config.admPanel = 1
config.absRefPrefix = /

page = PAGE
page.10 = HMENU
page.10.1 = TMENU
page.10.1.NO.wrap = <p>|</p>

Hints! You must navigate to the subpage using realurl! As soon as you change a setting in the admin panel you are redirected to the non realurl version of the page and the problem is gone.
