It is currently possible to create or modify sql tables using the tablesDefinitionIsBeingBuilt hook. This hook expects you to create a string with the syntax expected in ext_tables.sql.
There should be a way to directly access and modify the schema used by doctrine dbal since that would be a much simpler api than to modify a string.
