CSH Button visible even when context_help is disabled

Description
When the extension context_help is disabled, some CSH-Buttons are still visible.
One example is in Page Module (see screenshot). Clicking the link leads to a failed request.
Maybe it is enough if BackendUtility::cshItem does not generate the icon, if the extension is not installed.
Or maybe BackendUtility::wrapInHelp should check for it in addition to an installed cshmanual-extension.
