Missing index on sys_file_reference

Description
The table sys_file_reference has a missing index.
Like the table tt_content - there should be and index on the columns t3ver_oid,t3ver_wsid
The following select statement - that is generated during a copy of a sub-tree in the backend
could benefit greatly from that. In the test the statement was executed 136 times with a
using 1.7 seconds - as a comparison the same statement on the tt_content table took 189 ms.
