Use PSR-3 logger exclusively in ErrorHandler

Description
- Update ErrorHandler to use only the PSR-3 logger, not the legacy belog.
- Heavy cleanup of the ErrorHandler class along the way to make it more readable and maintainable.
- Introduce a new logging name/channel for php_errors.
