Remove ancient Extension Manager modules

Description
he Extension Manager interface has been rewritten in ExtJS for
TYPO3 4.5. This change removes the old modules, which duplicate the
functionality.

There should be some functionality under the hood still lying around that's not needed anymore. At least my patch removed more code than the one that has been committed did.

When installing a dependent ext from within another ext (opened in a lightbox window), old ext manager funcitonality appears. Tested with tinymce_rte that requires the removal of htmlarea. Also, the "window close" and "check dependency" buttons on newly opened windows still do not work (but that might be another bug report?).
