EXT:form - improve form-creation-wizard

Description
When creating a new form the "form-creation-wizard" is shown.
The steps of the existing wizard are partly irritating and can be optimised.

The odd thing now is the "simple" way helps you in no way. OTOH the so called "advanced" way does nothing more than present you a list of predefined forms.
From a users perspective it is not clear why you don't get the list always without the choice of "simple" and "advanced".
If we really need "simple" and "advanced" it shoule be the other way round. "simple" lets you choose a predefined form and "advanced" forces you to start from scratch.

For the record:
Whats you see within the steps depends on the configuration.
Step 1:
"form name"
This property is everytime visible.
"Form storage"
If there are more than one file storages configured, a select list is shown.
Step 2 (advanced):
"Form prototype"
If there are more than one showable prototypes are configured, a select list is shown.
"Start template"
If there are showable form boilerplate templates are configured, a select list is shown.
You can define boilerplate templates on a "per prototype" base. The content of the select list will change, if another prototype is chosen.
