Description
The fields alt. text, title and long URL desc. lack CSH (no icon at all).

The issue is very old, does this issue exists in newer versions of TYPO3 CMS (4.5 or 6.1)?
yes, still present in 4.5.20 and 6.1.0. More a change request than an issue IMHO
